The Health Education staff is available to speak at youth organizations, churches, schools, recreation centers, etc. on various health topics and services offered.  The Health Education Team, can provide educational opportunities in the form of programming, events, and presentations. Pamphlets and other educational materials are available

Presentation topics include:

  • Alcohol/Drug Education
  • Nutrition & Weight Management
  • Physical Fitness
  • Tobacco Education
  • Self-Care/ Hygiene
  • Healthy Relationships
  • Sexuality Issues/Sexually Transmitted Diseases (STDs)
  • Other Health Topics may be considered


  • Notify Health Education staff immediately if the event needs to be cancelled or re-scheduled.
  • Ensure that there will be sufficient participation for the program or event.
  • Complete a Health Education Program and Materials Request Form (90) days in advance of the planned event is REQUIRED

To schedule your health education program or presentation, or request to borrow materials please fill out the Health Education Program and Materials Request Form and submit by email or fax.

The Health Education Division will try to honor as many requests as possible. However, the availability of staff may prevent the Community Health Education Team from fulfilling all requests. In the event that we are not able to fulfill your request, the Health Education staff will recommend other alternatives in order to assist you

Please remember to complete form  at least 90 days prior to the event,  forms can be emailed, faxed, or mailed to Community Health Team.
