National Diabetes Prevention Program (DPP)

 

   

A year-long and  life-changing class that can help you lose 5-7% of your body weight and cut your risk for diabetes in half!  For 16 weeks, a trained lifestyle coach will work with you, one-on-one and in a group setting to help you learn simple changes for lowering your risk.  Discussions will include:

  • Stress Reduction
  • Improving Food Choices
  • Increasing Physical Activity
  • Using Coping Skills to Maintain Weight Loss

 


 
  • A trained lifestyle coach will guide and encourage you
  • Share support from other participants with the same goals
  • Incentives and Fun!
  • For KEHP members, the program is offered at no cost to those participating in the Kentucky Employees Health Plan. The program is offered at no out of pocket cost to those participating in the Kentucky Employees Health Plan. This is a covered health benefit.
  • For other insurance companies members, can contact their insurance company for cost.

Participants must be 18 years of age or older and have Body Mass Index of >24 
Participant provides a recent blood-based diagnostic test indicating pre-diabetes
Participant screens positive for pre-diabetes based on the CDC Prediabetes Screening Test.

                                                                                                                                                                           

Examples:

  • Fasting plasma glucose of 100-125 mg/dL
  • Plasma glucose measured 2 hours after 75 gm glucose load of 140-199mg/dL
  • A1C of 5.7—6.4
  • Diagnosed with Gestational Diabetes during pregnancy.
